how much epsom salt do you use per plant?

Mix one tablespoon of Epsom salts in a gallon of water, and apply to the root zone after planting.

it depends on the plant. for a garden start up it's one cup for 100 sq. ft. for tomatoes
and roses it's one teaspoon for each foot of plant height.
