Is this a sign of mental illness?

I know someone who has full conversations with themselves. They also have pretended to have friends in their head. This person is a adult. It is not normal for some one to do this.

Is this a sign of mental illness or a disorder?

    Sometimes normal people without any illness do conversations with themselves. The reason might be their deep emotions, including anger, nervousness, extreme focus or excitement. What I think these emotions are not constant, so if that behaviour doesn't change with time, then better to seek a professional.

    It's very normal to have an internal monologue that sometimes goes out your mouth... as well as continuing to have a rich fantasy life as an adult. Now, the disorder comes when there's distress or the person is having problems because of symptoms (hearing voices, behaviors that initiate police contact, anxiety, panic, depression, etc).

    It could be a big sign of schizophrenia. Having conversations with oneself at an adult age shows signs of hallucinations and one not. Talking to her about it would be a good idea, try to get her to elaborate on who she talks to. Maybe try to mention getting psychological help to her.
