Should I be concerned if my elastic waistband is wearing thin?
I mean, I don’t know how much longer the elastic waistband can keep my underpants from falling down.

Not normally a problem with underpants. It will just be a bit uncomfortable.
However, will be more of a problem if you wear a kilt.

Well apparently it is a thing to go underwearless these days, so you will soon be dressing in the height, or is it low, of fashion. Just don't bend over please.
